The GateKeepers Program
Our Purpose: To identify senior residents (60+) of Middlesex County in need of medical, social or mental health services.

Most seniors in our communities live successful independent lives. however, sometimes health or other issues may gradually create increasing isolation which places the individual at risk and threatens their independence.

There are a variety of community services available for our seniors, but we need your help to make a connection.

When you see changed behavior of conditions that cause you concern please call us to make a confidential referral.

Based on the urgency of the situation, prompt response will be undertaken by professionals to determine what help is appropriate. These practitioners will call on the individual to access the situation in such a way as to preserve the individual's dignity and Gatekeeper's confidentiality.

If you are concerned about a senior (60+) showing any of the following signs:

Physical Appearance
  • Dirty clothing or skin
  • Uncombed or dirty hair
  • Inappropriate clothing
  • Strong personal odors
Mental/Emotional State
  • Forgetful or confused
  • Impaired reasoning or illogical thought
  • Depressed, angry or anxious
  • Delusional
  • Confused about money
  • Socially isolated
Physical Changes
  • Diminished eyesight, hearing or mobility
  • Incontinence
  • Dramatic weight gain or loss
Living Conditions
  • Deteriorating living conditions
  • Strong household odors
  • Uncollected mail or newspapers
  • Neglected pets
Referrals should be made during regular business hours- weekdays from 9:00 a.m. to 4:00 p.m.

A trained coordinator will take the necessary information. Please be prepared to indicate this is a Gatekeeper referral and provide:
  1. The Senior's name and address (and phone number if possible)
  2. A brief description of the problem or situation
  3. your name and phone number in case we need to reach you for additional information
All referrals are held in strict confidence
